Pills Guide#
Pills are the consumables in The Binding of Isaac that test your memory the most: they differ only by color, yet their effects are reshuffled every run. Unlike cards, you cannot judge what a pill does from its appearance — you can only map colors to effects by testing. This page compiles the real effects of every pill in the current version (including the Repentance DLC), the Horse Pills enhanced variants, and the related synergy items, for quick reference while testing. Card and rune effects are constant across runs — see Cards and Runes Collection Guide.
Core Mechanic: Colors and Effects Are Randomized Every Run#
The pill system’s basic rules are completely different from cards; internalize these before testing:
| Rule |
Explanation |
| Shared slot |
Pills and cards share the same Pocket slot, used with Q; you can hold only 1 pill or 1 card at a time |
| Random assignment |
At the start of each run, effects are randomly assigned to colors and never change for the rest of the run |
| Rerolled across runs |
In a new run, the color-to-effect mapping is completely reshuffled; last run’s memory does not carry over |
| First use reveals |
Once a color has been used 1 time, that color’s effect is publicly shown for the rest of the run; later same-color pills can be used with confidence |
| Negative reduction |
Negative pills of a given type are about 80% as strong as the corresponding positive pill |
“First use reveals” is the key to the whole system: the testing cost is paid only once, and after confirmation same-color pills become reliable consumables with known effects. The “negative reduction” can be verified directly with the Speed pills — Speed Up grants +0.15 movement speed while the corresponding Speed Down only takes 0.12, the latter being about 80% of the former. This reduction rule makes the expected loss from testing smaller than many people imagine.
Testing and Identification Strategy#
Since colors are reshuffled every run, you must rebuild the color-to-effect table each run. Here is the lowest-risk testing workflow:
| Strategy |
Explanation |
| Clear the room before testing |
Clear all enemies in the current room before eating an unknown pill; one-shot damage like Bad Trip or movement lockout like Paralysis won’t kill you mid-combat |
| Test at full health first |
At full health a Bad Trip only costs 1 full heart; and with 1 or fewer red hearts left, Bad Trip automatically becomes Full Health — free healing |
| Use confirmed pills immediately |
Stat pills (Speed Up, Tears Up, etc.) should be eaten right after confirmation; they have no timing window and merely block the pocket slot from new consumables |
| Hold healing pills in reserve |
Full Health, Balls of Steel and other healing effects give the most value saved for low health or before a Boss fight |
| Remove negatives quickly |
Once a pill is confirmed negative (Health Down, stat drops, etc.), use it up somewhere safe as soon as possible to free the slot; with only 1 red heart container left, Health Down becomes Health Up and can save a dying run |
| Remember the color grid |
The reveal mechanic tracks colors, not counts; every pill of the same color has the same effect, so once confirmed, pick up and use on sight |
Complete Pill Effects Table#
The effects below are confirmed data for the current version, grouped by use. The English names in the Pill column are the in-game originals.

| Tears Up |
Tears +0.35 |
The most solid fire-rate buff; eat revealed ones on sight |
| Tears Down |
Tears -0.28 |
Clearly drags down output; high risk during the testing phase |

Range +2.5 |
Clearly helps with safe positioning |
| Range Down |
Range -2 |
Forces point-blank output; avoid before a Boss fight |
| Luck Up |
Luck +1 |

| Luck Down |
Luck -1 |
Affects all drop rolls; negative but a manageable magnitude |
Health and Resource Pills#
| Pill |
Effect |
Usage advice |
| Bad Trip |
Deals 1 full heart of damage; automatically becomes Full Health at 1 red heart or fewer |
A counterintuitive safety net — at low health it is actually a healing pill |
| Full Health |
Restores all red hearts |
A lifesaver at low health; a pure waste at full health |
| Health Up |
+1 empty red heart container |
Permanently raises max health; positive all run |
| Health Down |
-1 red heart container; becomes Health Up when only 1 container remains |
Eating it early cuts your own max health; mind the container fallback conversion |
| Hematemesis |
Drains red heart containers down to 1 and drops 1-4 full red hearts |
Trades containers for hearts; be careful at high container counts — dropped hearts cannot be stolen by enemies |
| Balls of Steel |
+2 Soul Hearts |
The Horse Pill version gives +4 directly — a top-tier defensive tool |
| Bombs Are Key |
Swaps your bomb and key counts |
Check what you lack before eating; the Horse Pill version adds +50% to both sides on swap |
| 48 Hour Energy |
Drops batteries and fully charges the current active item |
An excellent partner for charge-based items |
Combat and Summoning Pills#
| Pill |

Usage advice |
| Bad Gas |
Releases a poison cloud that damages nearby enemies |
Good for breaking a close-quarters siege; useless against poison-immune enemies |
| Explosive Diarrhea |
Drops 5 lit bombs behind you over 5 seconds |
A moving minefield on your retreat route; don’t step on them yourself |
| Lemon Party |
Creates a large yellow puddle underfoot; enemies standing in it take damage |

| Pheromones |
Charms all enemies in the current room |
A truce banner for dangerous rooms — the more enemies, the better |
| Pretty Fly |
Grants 1 orbiting fly familiar |
Pure buff; blocks shots and adds damage |
| Friends Till The End! |
Spawns 3 blue flies to fight for you |
Free summons — an offensive pill |
| Power Pill |
Triggers the effect of the Gamekid item |
A short burst; eating it before entering a room yields more |
| Infested! |
Spawns blue spiders to fight for you |
Blue-spider related; exact trigger conditions per in-game behavior |
| Infested? |
Spawns blue spiders to fight for you |
A sister pill to Infested!; the two have different trigger conditions, per in-game behavior |
Exploration and Field Pills#

Effect |
Usage advice |
| I Can See Forever |
Opens the entrances of all Secret Rooms and Super Secret Rooms on the floor |
The best-value exploration pill in the game; eat it on sight |
| Telepills |
Randomly teleports you elsewhere, with a small chance to enter the I AM ERROR Room or Black Market |
Half a random-teleport ticket; gambling for the I AM ERROR Room is its hidden value |
| Amnesia |
Turns the floor’s map info into question marks |
Hides exploration progress; map-revealing items counteract it |
| I Found Pills |
Cosmetic change only, no real effect |
A pure show pill; wastes a slot but is harmless |
Status Change Pills#
| Pill |
Effect |
Usage advice |
| Paralysis |
Prevents movement for a short time |
Very dangerous if hit mid-combat; always test in a safe room |
| R U a Wizard? |
Tears fire diagonally for about 1 minute |
Massively changes your attack pattern; you must re-learn the trajectories |
| One Makes You Small |
Shrinks your body, shrinking your hitbox accordingly |
A bullet-hell lifesaver; the smaller hitbox is a real benefit |
| One Makes You Larger |
Enlarges your body |
Easier to hit — a negative effect |
| Retro Vision |
Pixelates the screen |
Affects judgment, not stats; pure visual interference |
| Puberty |
Grows hair on your appearance |
After eating 3 in total, unlocks the Adult transformation |
| Re-Lax |
Leaves poop behind as you move |
Can block paths or pair with poop-exploding items |
Challenge-Exclusive Pills#
The following two pills only appear in Challenge #24:
| Pill |
Effect |
Usage advice |
| Addicted |
+1 damage taken in the current room |
A challenge-exclusive negative pill; weigh it before entering |
| Percs |
Grants The Wafer effect in the current room |
A challenge-exclusive positive pill; great for reducing damage |
Horse Pills: Enhanced Pill Variants#
Horse Pills are the oversized pill variant added in Repentance, bigger and more visually distinct. The rule is simple: same color and same effect as the normal pill, but fully enhanced in strength. Confirmed comparisons:
| Pill |
Normal version |
Horse Pill version |
| Balls of Steel |
+2 Soul Hearts |
+4 Soul Hearts |
| Bombs Are Key |
Swaps bombs and keys |
Adds +50% to both sides on swap |
| Full Health |
Restores red hearts |
Restores red hearts, plus +3 Soul Hearts |
The enhancement amounts of the remaining Horse Pills follow in-game behavior; the direction is always enhancement, never nerf. Eat the bigger pill on sight — it will never hurt you.
Items That Synergize with Pills#
| Item |
Synergy with pills |
| PHD |
Auto-identifies pill effects on pickup and converts negative pills into positive effects — the testing mechanic is bypassed entirely |
| Lucky Foot |
Removes the negative effects of stat-down pills, greatly reducing testing risk |
| Virgo |
Rerolls the current pill, turning a bad pill in hand into a second draw |
| Safety Cap |
Increases the pill drop rate; more pills mean faster reveals |
PHD is the qualitative-changer of the pill system: with negatives converted to positives, every pill becomes a pure-gain consumable, and a pill-focused playstyle is finally viable.
Common Misconceptions Cleared Up#
| Misconception |
Actual rule |
| A certain color is always a bad pill |
The color-to-effect mapping is reshuffled randomly every run; color itself predicts nothing, and last run’s experience does not carry over |
| Bad Trip poisons you and drains health over time |
Bad Trip is a one-time 1 full heart of damage; it applies no poison and has no lingering drain |
| Revealed effects carry over across saves |
Reveals only last within the current run; everything is reshuffled next run and must be re-tested |
| Negative pills are as strong as positive ones |
A negative pill of the same type is about 80% as strong as the positive counterpart: Speed Up +0.15 corresponds to Speed Down -0.12 |
| Eating Health Down is always a loss |
With only 1 red heart container left it automatically becomes Health Up — a built-in fallback conversion |
Strategy Summary#
- Testing discipline: the first few pills of every run are always eaten in a cleared safe room; confirm effects before deciding whether to keep, use, or swap the rest;
- Slot trade-offs: pills and cards share the pocket slot — think before picking up; an unidentified pill is not necessarily better than a known card in hand, but revealed good pills deserve priority;
- Stat pills have zero timing cost: Speed Up, Tears Up, Range Up and Luck Up carry no timing considerations once confirmed — just eat them;
- Time healing pills right: hold Full Health, Balls of Steel and Bad Trip (fallback triggers at low health) for low health or before a Boss fight;
- Use exploration pills unconditionally: I Can See Forever opens every Secret Room entrance on the floor — the best-value pill in the game; use it the moment you get it;
- Handling negatives: use Health Down as soon as possible while containers are plentiful; Luck-down pills can be kept to be neutralized by a later Luck Up.
