The Binding of Isaac: Repentance Endgame Guide#
The endgame of The Binding of Isaac is a long-haul project that runs from Mom’s Heart all the way to Dead God and full completion: first you clear the base route and its two branches, then unlock Mega Satan, the alternate route added by Repentance, and the final route — with Hard Mode, Greed Mode, and a batch of special-rule runs mixed in along the way. This page lays out that whole web in true progression order and breaks down what Hard Mode actually changes — it’s far more complex than “enemies get stronger,” and the real killers hide in spawn rules and the resource economy
Endgame Route Overview#
The endgame progression chain is fixed, and every step opens the door to the next. The table below lists each stage’s goal and how to get there, in order — for exact trigger details, defer to the game itself
| Order | Goal | Notes |
|---|---|---|
| 1 | Mom’s Heart | The final boss of the base route — the core objective of every run |
| 2 | It Lives | After you beat Mom’s Heart several times, this boss fight upgrades into It Lives — the exact count is determined in-game |
| 3 | Cathedral / Sheol | Beating It Lives opens two endgame branches — the Cathedral is the angelic route, Sheol the demonic one |
| 4 | Isaac → ??? | Cathedral line: beat Isaac first, then enter the Chest floor to face ??? (Blue Baby) |
| 5 | Satan → The Lamb | Sheol line: beat Satan first, then descend into the Dark Room and defeat The Lamb |
| 6 | Mega Satan | Requires two Key Pieces to open the door — see the unlock guide for how to get them |
| 7 | Delirium | Fought deep inside The Void — a monster once every DLC route is stacked together; see the boss guides for how to fight it |
| 8 | Mother | The endpoint of Repentance’s alternate route (Alt Path) — see alternate routes for the way there |
| 9 | The Beast | The final showdown upstairs in Home — the closing fight of Repentance’s true ending |
| 10 | Dead God | The ultimate full-collection achievement — see the unlock guide for long-term planning |
Hard Mode#
Hard Mode isn’t a matter of simply bumping enemy health up a tier — enemy HP does not universally increase. The real difficulty comes from a systematic tightening of spawn rules, drop economy, and gambling payouts. The table below shows Hard Mode’s actual differences, and every one of them directly reshapes how you manage a run
| Mechanic Change | Specific Difference |
|---|---|
| Room generation | 2-3 extra rooms spawn per floor — fewer easy rooms, more difficult ones, and each floor takes noticeably longer and costs more |
| Champion enemies | Spawn chance rises from 5% to 20%; meanwhile champion drop chance falls from 100% to 33% — elites are more common but far worse value |
| Red heart drops | Red heart drop chance after clearing a room is lowered — roughly one-third of drops are removed; normal bosses no longer drop red hearts on defeat, only champion bosses do |
| Curses | Cursed floors show up more often — route planning and early-run rhythm get disrupted far more easily |
| Gambling and begging | Slot Machines pay out half as much; Blood Donation Machines produce only 1 coin per use; Beggars need 6 donations before they start giving anything back |
| Soul heart acquisition | Blue and purple fires drop soul hearts 10% of the time, down from 25%; Double Heart drops fall to roughly 1.69% |
| Shops and terrain | Shops have a 50% chance of being downgraded; Tinted Rocks appear 50% less often |
| Enemy strength | Some enemies and bosses attack and move faster — but enemy health does not universally increase |
Hard Mode Survival Tips#
Stack these changes together and running a run looks nothing like Normal Mode:
- HP planning beats damage output — normal bosses drop no hearts and room heart drops are cut by about a third, so running a red heart deficit is the norm; the value of soul-heart and healing items rises across the board
- Don’t treat gambling as an income stream — slot machines pay half and blood donation machines give out 1 coin a pull; on Hard Mode both machines are basically down to entertainment value
- Shops can be downgraded — the same key is usually better spent on the item pedestal in the Treasure Room than gambled on a shop that might downgrade, so allocate resources more conservatively
- Save your slack for long floors — 2-3 extra rooms per floor means more combat rounds; taking less damage in the opening stages decides how far a run goes far more than picking up one more damage item
Hard Mode Exclusive Unlocks#
Hard Mode has a batch of exclusive unlocks, the heaviest of which is Godhead — one of the longest single unlock grinds in the entire game:
| Unlock | Requirement |
|---|---|
| Godhead | Earn the completion marks on Hard Mode with all 17 regular characters |
| Smelter | Hard Mode exclusive — defer to the game for the exact requirement |
| Mega Mush | Hard Mode exclusive — defer to the game for the exact requirement |
| Death Certificate | Hard Mode exclusive — defer to the game for the exact requirement |
| Bethany | Beat Mom’s Heart / It Lives on Hard Mode as Lazarus without dying |
A mark requirement spanning 17 regular characters makes Godhead one of the biggest projects of the endgame — tainted characters track a separate list of progress, see Tainted Characters; for the full table of unlock conditions per character, see the unlock guide
Completion Marks#
Completion marks are the basic unit of endgame progress — each character leaves a mark for every route and special run, and you can check your collection status on the character screen. The mark list:
| Mark | Target | Notes |
|---|---|---|
| Mom’s Heart | Beat Mom’s Heart / It Lives | Base route finish line |
| Isaac | Beat Isaac | Cathedral line finish |
| Satan | Beat Satan | Sheol line finish |
| The Lamb | Beat The Lamb | Dark Room route |
| Boss Rush | Clear Boss Rush | A time-limited challenge room — refresh conditions per the game |
| Hush | Beat Hush | Hidden boss of the ??? (Blue Womb) floor — entry conditions per the game |
| Mega Satan | Beat Mega Satan | Two Key Pieces open the door |
| Delirium | Beat Delirium | The Void route |
| Mother | Beat Mother | Repentance alternate route |
| The Beast | Beat The Beast | Upstairs in Home — Repentance true ending |
| Greed | Finish Greed Mode | Endpoint boss is Ultra Greed |
| Ultra Greed | Finish Greedier | Hard version of Greed Mode |
Collect every mark above on one character, then switch to the next and repeat — once all 17 regular characters have a full set of Hard Mode marks, Godhead is yours. For a breakdown of each route’s boss mechanics, see Bosses and Elites
Greed Mode#
Greed Mode is a wave-based challenge mode that runs parallel to the main story — you push floor by floor with shop restocks between floors, and it ends when you beat Ultra Greed; the harder version is Greedier, where wave pressure is higher. For full wave strategies and coin management, see the Greed Mode guide
Challenges and Daily Runs#
Challenges rebuild the entire run around a fixed starting item and special rules, unlocking a matching reward on completion; Daily Runs hand every player the same fixed-seed start and compare scores on a leaderboard. The full challenge list and scoring rules defer to the game — for dedicated coverage, see Challenges and Daily Runs
Endgame Long-Term Progression Order#
There is no single correct order for full completion, but the following progression is the least painful:
- Clear the base routes first — use a comfortable character to sweep up base marks like Mom’s Heart, Isaac, Satan, and The Lamb while banking character unlock progress
- Advance characters and marks together — the more characters you unlock, the more starting options you have, and the smoother the later mark-grinding curve gets
- Re-earn the marks on Hard Mode — the goal is Hard-exclusive unlocks like Godhead and Smelter; see build ideas for itemization thinking
- Fill in special runs and the Repentance routes — wrap up Greed Mode and the challenges, then dive into the alternate route to take on Mother and The Beast
- Sprint for Dead God — the grand tally of marks, items, and trinkets; see the unlock guide for endgame planning
The pacing of each stage swings wildly with your build and your luck — there’s no need to rush on someone else’s schedule; every run pays something back
Endgame FAQ#
| Question | Answer |

Summary#
Isaac’s endgame is a web that keeps expanding: every new character unlocked adds another way to play the routes you’ve cleared, and every new route pushed adds a row of targets to the mark sheet. Learn Hard Mode’s real rules first, then plan your own progression order — and leave the rest to the run count
