Full Achievement Walkthrough#

Companion Quests#

Every companion has a personal questline that runs through all three acts. Shadowheart’s quest involves the Sharran temple and the choice around the Nightsong, Astarion’s involves the vampire lord Cazador, Gale’s involves the Crown and Mystra, Lae’zel’s involves the githyanki queen and Voss, Karlach’s involves her infernal engine, Wyll’s involves his pact with Mizora, Halsin’s involves lifting the shadow curse, and Minsc and Jaheira join in Act 3. Minthara requires choosing to attack the grove in Act 1.

Boss Kills#

Boss achievements unlock the moment the kill lands. Raphael and the hidden red dragon Ansur are the two fights worth preparing for well in advance — the rest fall in line as you push through the main story.

Missable Achievements#

Achievement When It’s Missed Tip
Minthara Act 1 choice You must choose to attack the grove in Act 1 to recruit Minthara. Side with the grove and she cannot be recruited
Save the Refugees End of Act 1 Resolve the tiefling refugee crisis before leaving Act 1, and make sure they all survive
Resist the Dark Urge Entire run A Dark Urge playthrough requires picking the resist option every single time — giving in even once fails the achievement
Mind flayer transformation Act 3 Requires using specific tadpoles/brands and completing the transformation at a specific location in Act 3
Every long rest event Entire run Many long rest events only trigger at specific story points; skipping camp for long stretches costs you plenty of scenes
The Iron Throne After Gortash dies in Act 3 The Iron Throne starts going up in flames after Gortash’s death — rescue every prisoner within a limited number of rounds
All Act 1 dungeons Before Act 1 ends The Underdark has multiple entrances (the goblin camp, the apothecary’s cellar, and more); miss one and it can’t be picked up after Act 1 ends

Strategy Details
Party comp Suggested: 1 tank + 1 healer + 2 damage dealers (e.g., Fighter + Life Cleric + Sorcerer + Paladin)
Long rest management Long rest resources are limited on Honour Mode — lean on short rests for recovery first
Set up early Use invisibility, flight potions, and Haste to get into position before a fight starts
Use the environment Shove enemies off cliffs, exploit oil barrels and explosives, and claim the high ground
Builds Paladins bring big burst damage out of the box, sorcerers twin their spells, Life Clerics heal reliably
Backup saves On Honour Mode, press F5 to save manually (even with a single save slot, you can still save your current progress)

FAQ#

Question Answer
How many playthroughs for all achievements? At least 3: a normal Balanced first run, a second run as the Dark Urge with opposite choices, and a third on Tactician/Honour Mode.
Which achievement is the hardest? Completing Honour Mode. If the party wipes, the save is permanently deleted and you start over from the very beginning. Zero room for error.
What classes are recommended for Honour Mode? Paladin-Warlock-Sorcerer combos (spellblade dark knight), Life Domain Cleric (reliable healing), and Open Hand Monk (high mobility, high burst).
Can Minthara be redeemed? No. She only joins after you choose to attack the grove — she’s the evil route’s reward companion. You can, however, steer her toward good in later story beats.
Is there a time limit for the Iron Throne rescue? Yes. You must rescue every prisoner (about 8-10 in total) within 6 rounds. Haste and teleportation spells are highly recommended.
Can you revert after becoming a mind flayer? No. Becoming a mind flayer is permanent — appearance and some dialogue change. But it is an ending style of its own.
How many endings are there? More than 10 variants across 5 base ending types, depending on your allies, which companions survive, and your final choices.
