Tainted Characters Guide#

Tainted Characters Guide

In the Repentance DLC for The Binding of Isaac, each of the 17 base characters has a “mirror version” — the Tainted Characters. They are not reskinned stat boosts, but complete redesigns that push each character’s core gameplay to an extreme: how you take items, how health works, and how you deal damage are all rewritten. Difficulty is generally higher than the corresponding regular character, and the playstyle ceiling changes accordingly.

This page breaks down each tainted character’s unlock method and core mechanics, with the corresponding strategy for each. Each character’s mechanics were checked against the official English Wiki; any detail without solid confirmation is marked “per in-game values” — no unfounded assertions.

One more thing to make clear up front: tainted characters use non-standard health structures extensively — consumable hearts, one-hit kills, and black heart systems all appear, and health design differs drastically between characters, so no single “starting health table” can cover them. This page therefore has no health number table; all health-related design is folded into each character’s mechanics breakdown, with specific values per in-game values.

Unlock Method: Identical for All 17#

The unlock method is exactly the same for all 17 tainted characters, but each one must be done separately:

  • Reach the Home floor with the corresponding regular character;
  • Open the hidden closet with the Red Key to unlock that character’s tainted version — before the Red Key is unlocked, certain related effects can also open the closet, per in-game values;
  • Note the word “corresponding”: unlocking Tainted Cain requires opening the closet with Cain himself; other characters won’t work;
  • For unlock conditions and prerequisites of all 34 characters, see the Unlock Guide.

Tainted Isaac#

Tainted Isaac starts with no items, and his passive mechanic rewrites “picking up items” itself: each time you take an item, a second item spawns on the pedestal for you to choose between; meanwhile the item bar keeps only 8 slots — once you exceed the cap, the earliest item you picked up is automatically dropped.

This mechanic turns “picking up” from pure gain into a decision. The old habit of grabbing everything on a pedestal must go: 8 slots cannot accommodate hoarding, and every slot must be spent where it counts.

Strategy:

  • Compare the spawned second item before picking — treat it as a free pick-one-of-two; look at what both items are before deciding;
  • The item bar is a hard cap — save slots for core items that define your build, and willingly pass on scattered low-value items;
  • Overflow drops are irreversible — when the 9th item walks in, the one pushed out is the earliest you took; factor that into your pickup order;
  • Every run’s item structure differs, so instead of memorizing build recipes, put your energy into evaluating item value — this mechanic is essentially a test of item-pool mastery.

Tainted Magdalene#

Her starting item is Yum Heart (a healing active item). Her health system is completely rebuilt: red heart containers are no longer permanent — hearts are held as consumables that decay; as compensation, she moves very fast.

This means the traditional “stockpile health” approach fails entirely on her: hoarding hearts is pointless because they rot. Her high speed shifts her survival model from “tanking with health” to “dodging with positioning”.

Strategy:

  • Treat speed as your primary defense — dodge bullets with positioning instead of relying on a health bar to absorb damage;
  • Plan the use of consumable hearts as soon as you get them; don’t let them rot in inventory;
  • Fold Yum Heart into your regular healing rhythm; don’t wait until your health bar is nearly empty (healing details per in-game values);
  • Playstyles tied to red heart containers (such as the cost structure of Devil Room deals) must be recalculated for her; specific rules per in-game values.

Tainted Cain#

His starting item is the Bag of Crafting: collect up to 8 pickups into the bag to craft an item. Output strength is decided by the overall quality of the pickups in the bag — there are no fixed recipes.

This deserves its own emphasis: the “material-list recipes” circulating online (a certain set of pickups in fixed quantities guaranteed to craft a specific item) are unreliable. Output is not looked up from a table — the quality composition of the 8 pickups in the bag is the deciding factor. Tainted Cain’s core loop is “amassing quality”, not “memorizing recipes”.

Strategy:

  • To craft high-strength items, prioritize looting and hoarding high-quality pickups, then craft once you have a full bag;
  • Use low-value pickups for emergencies or transitions; don’t be stingy about them;
  • Bag capacity is limited — if you let 8 slots fill without crafting, your collection rhythm stalls — manage the “collect and craft” cycle well;
  • Every pickup on the map is potential material; plan looting routes around material value, not just item pedestals.

Tainted Judas#

His starting item is Dark Arts: activate for a short dash — the character is briefly invincible during it, and enemies along the dash path take piercing damage; each kill shortens the item’s cooldown.

It is not a passive form change triggered on hit, but an offensive tool you time yourself — invincibility is just the dash’s built-in safety; the piercing damage is the real payload.

Strategy:

  • Use Dark Arts as an offensive skill — when the cooldown is up, find a target instead of clutching it for safety;
  • Use the invincibility frames to cross dense bullet patterns and close distance — this is his standard answer to being swarmed;
  • The kill-shortens-cooldown mechanic rewards continuous combat: the faster you clear a wave, the faster the next dash returns — the rhythm snowballs;
  • Plan dash direction in advance — the invincibility window is short; dashing the wrong way is a free death.

Tainted ???#

His starting item is Hold: swallow pickups from the floor to store them inside, then spit them out after clearing the current room.

Pickups in his hands shift from “instant effect” to “storable resource” — when to swallow, what to swallow, and what to spit out after clearing rooms form his core loop.

Strategy:

  • Evaluate room pickups before swallowing — being swallowed does not mean instantly converted to value;
  • Room-clear spit-out is a fixed settlement rhythm; decide what this wave will store before entering the next door;
  • Treat storage as a hand of cards rather than a warehouse — timing the spit-out should sync with the fight rhythm (specific spit-out rules per in-game values).

Tainted Eve#

Her starting item is Sumptorium: sacrifice red hearts to spawn clot clones that fight for you.

On her, red hearts are “life” and “soldiers” at once for the first time — hearts given become board presence, hearts kept are your margin for error. The trade-off between the two runs through her entire game.

Strategy:

  • Count your health line before sacrificing — no matter how lively the board, if the body falls, everything is lost;
  • Treat Sumptorium as a resource converter that “trades blood for board presence”: the greater the pressure, the higher the conversion value;
  • The heal-and-sacrifice cycle is her core loop — your healing rhythm directly determines your board forces (clot clone stats per in-game values).

Tainted Lazarus#

His starting item is Flip: actively switch between two forms, with both forms sharing damage gains.

Unlike regular Lazarus’s passive “switch on death” mechanic, the tainted version hands the switch’s initiative entirely to you — when to press Flip and which form to switch to are your calls.

Strategy:

  • Switch timing being yours means Flip is a rhythm tool — decide which form will fight before entering the boss room;
  • Shared damage removes any fear of “losing out by switching” — switch boldly and focus on how to fight after the switch (specific sharing rules per in-game values);
  • Learn each form’s differences through practice — unavoidable homework for playing him well.

Tainted Eden#

Every time he enters a new room, his owned items are randomly rerolled — the build is always in flux.

He is not the ordinary random character of “random start, stable middle”, but an extreme design where even the build in hand keeps shuffling. Fixed recipes mean nothing on him; every room entry is a fresh evaluation.

Strategy:

  • Give up long-term build planning; focus on “how do I fight with this current set”;
  • Check the current item state upon entering each new room before deciding combat and routes;
  • Item-pool mastery sets his floor — the more complete your knowledge, the calmer you handle each reroll (specific reroll behavior per in-game values).

Tainted Lost#

No health — any single hit ends the run; and there is no holy card shield. The lack of that safety net is exactly what separates him most from regular Lost.

In other words, the small insurance regular Lost still has is stripped away entirely — he is a pure test of positioning and decision-making, with zero margin for error.

Strategy:

  • Treat every room like a boss room for positioning — one mistake is one run over;
  • Route planning comes before gains — avoiding high-risk rooms is nothing to be ashamed of; staying alive is your damage output;
  • Don’t count on mechanics to save you — with no death-defying insurance, your play is the only shield.

Tainted Azazel#

No starting item; Brimstone is no longer “one item in the bar” but an innate short-range laser — range compressed to be very short; the health system is built around black hearts.

Like regular Azazel, he fights at point-blank range, but the tainted version compresses range even harder and swaps the whole health structure — regular-character experience only helps you halfway.

Strategy:

  • Create combat distance yourself — short range means every engagement requires closing in proactively;
  • Black heart trade-off details per in-game values, but the principle is universal: health itself is a resource; don’t waste it;
  • The innate Brimstone is your early-to-mid game damage anchor — plan the first few floors’ pacing around it.

Tainted Apollyon#

His starting active item is Abyss: destroy items in the room, converting them into a swarm of locusts that fight for you.

Once an item is fed to the Abyss it is irreversible — his damage system shifts from “amassing a build” to “constant trade-offs between items and the swarm”.

Strategy:

  • Ask of every item: hold it and use it, or turn it into locusts? This is his core decision;
  • Don’t sacrifice key survival items easily — the swarm cannot block every bullet for you;
  • Abyss output details per in-game values, but the direction is clear: trade items for power, and commit to the choice.

Tainted Bethany#

The active item charge system is replaced wholesale: no more conventional charging — active items run on soul charges.

Soul charges become the “ammo” for active items — soul-heart-type resources, in her hands, are upgraded from a pure survival buffer to skill fuel. Survival and output share one resource pool, and every charge must be spent carefully.

Strategy:

  • Active item usage frequency is directly constrained by soul charges — check how much “ammo” remains before pressing the skill;
  • The specific soul-charge and soul-heart conversion rules per in-game values, but the operating principle is clear: fill resource gaps first;
  • Her power curve depends on soul charge management — the rhythm between hoarding and spending takes practice to master.

The Remaining Five Tainted Characters#

Tainted Samson, Tainted Lilith, Tainted Keeper, Tainted Forgotten, and Tainted Jacob/Esau — for these five, this page lacks reliable sources on their per-character mechanics; leaving them blank beats writing them wrong, and all specific mechanics defer to in-game values. What is certain: their unlock method is identical to the rest — open the hidden closet on the Home floor with the Red Key using the corresponding regular character.

Leaving gaps is not laziness: tainted characters’ mechanics are rewritten so drastically that describing them from memory only manufactures wrong guides. For these five, verify gameplay details against actual in-game behavior.

Why There Is No Unified Health Table#

As stated at the top of this page, once more explicitly: tainted characters’ health designs mostly deviate from the standard “N red heart containers” structure — consumable hearts rot, one-hit kills exist, and black heart systems each follow their own logic, with huge per-character differences. Any “tainted character starting health table” risks misleading readers, so this page folds health notes into each character’s mechanics breakdown; specific values per in-game values.

Final Words#

The 17 tainted characters share one unlock flow yet offer 17 playstyles pushed to extremes: Isaac’s slot management, Cain’s crafting economy, Eve’s health conversion, Lost’s zero-margin positioning — each forces you to rethink the concepts of “resources” and “margin for error”. Regular-character fundamentals remain the foundation here, but what gets built on that foundation is a completely different house.

  • To solidify regular-character fundamentals first, head back to the Characters Guide;
  • For unlock prerequisites and conditions, check the Unlock Guide;
  • For build ideas, see Builds and the Item Encyclopedia — tainted systems often need a fresh start, but the underlying logic of item value carries over;
  • For the payoff structure of Devil and Angel Rooms, see Devil Room and Angel Room Mechanics — some tainted characters’ health systems change how these deals are priced; specifics per in-game values.
